The short answer
The short answer: if sagging of the SMAS layer is your main concern, Ultherapy comes first; if dermal laxity and fine lines are the main concern, Thermage comes first. The two are not competitors — they act on different layers — and when sagging and laxity coexist, they are sometimes combined.
Which layer does each treatment act on?
Ultherapy delivers high-intensity focused ultrasound (HIFU) down to the SMAS layer about 4.5mm beneath the skin — the thin membrane that supports the face — contracting the lax support structure itself. It addresses changes that feel like the face has 'come down': jawline and cheek sagging.
Thermage delivers monopolar radiofrequency (RF) broadly across the dermis, inducing collagen contraction and regeneration. It addresses the feeling that skin has 'thinned and loosened' — fine lines and firmness around the eyes.
What decides the choice?
The deciding factor is not a device's reputation but the layer of the sagging. At consultation we assess skin thickness, fat volume, and the degree of SMAS-layer descent to determine the layer.
For a fat-driven double-chin type of sagging, a fat-targeting device such as InMode may come first. The layer diagnosis is, in effect, the device selection.
Is combining them more effective?
Because the layers differ, combining them is logically sound, and we do build plans that manage the SMAS (Ultherapy) and the dermis (Thermage) together. That said, not everyone needs both — for many patients, one is enough.
